Confidentiality of Group Members

I acknowledge that I may learn the identities of other group members and that I am required to keep any and all identifying information confidential.

Limits of Confidentiality

I understand that the Change Creates Change Team will only collect, use or disclose my personal information with my express or implied consent, unless a collection, use or disclosure without consent is permitted or required by law.

I understand that the Change Creates Change Team may be required by law to disclose my personal information in any of the following circumstances: a) There is an imminent danger to an identifiable third party or to self; or b) When there is reasonable cause to suspect abuse or neglect of a child.

Email Consent

I give consent to the Change Creates Change Team to communicate with me via email to provide reminders, share electronic resources and for scheduling purposes.

I understand that: a) Information of a sensitive or urgent matter will not be communicated via email, and b) Either party may stop communication via email at any time for any reason.

I am accepting the following risks associated with email: a) Email is not a secure or confidential form of communication. Unencrypted email messages that are sent across the internet could be potentially be intercepted and read by unintended parties, and b) Email messages may be delayed for technical reasons or be accidentally sent to spam or trash folders.
