Transferring, Sharing or Selling Materials

I acknowledge that any materials provided are owned by Change Creates Change Eating Disorder Care and it is not acceptable to be transferred, copied or sold to others.

Sharing of Information

I authorize the Change Creates Change Team to collect and use my contact information to (a) notify me of new services or goods available at Change Creates Change, and (b) to notify me of special events and opportunities available at Change Creates Change.

Program Length and Fees

PARENTS EMPOWERED PROGRAM: I understand that my credit card will be charged 30 days following the initial charge, due immediately upon enrollment. I understand that while the program is 12 months long, I can elect to withdraw after the minimum 3 month term by emailing [email protected] at least 7 days before the next automatic charge.

EATING DISORDER CARE FOR PCPS: I understand that my credit card will be charged the full purchase price immediately upon enrollment. 

I understand that I am responsible for all harmonized sales tax (HST), provincial sales tax (PST), goods and services tax (GST), value added tax, use and excise taxes, and any other similar taxes, duties and charges of any kind imposed by any federal, provincial, territorial or local governmental entity on any amounts payable.

I understand that refunds are not available.

I understand that I will only have access to program content throughout the period of enrollment. 

Withdrawal

I understand that I can withdraw my consent at any time by contacting the Change Creates Change Team at [email protected].
